Cover your health out-of-pocket

Get a DKV (LALUX Group) quote tailored to your age and needs — hospitalisation, dental, optical — in minutes.

What does the CNS reimburse, and what do you still pay?

Why supplementary cover, when the CNS already reimburses?

In Luxembourg, every resident and worker is affiliated to the CNS (National Health Fund), which reimburses most everyday care — often at an excellent level. But it leaves an out-of-pocket on several items: in hospital, a personal contribution of €26.14 per day (2025 rate, excluding under-18s, capped at 30 days per year), and above all the private room and fee supplements, which are not covered. Advanced dental (prostheses, implants), optical, refractive surgery and alternative medicine are only partially covered. There is, however, a safety net: once your accumulated out-of-pocket for the year exceeds a set amount, the CNS reimburses the excess on request — this personal-contribution ceiling is set at €518.18 in 2026 (versus €509.68 in 2025 and €497.38 in 2024). Note too an indexation of the agreed tariffs on 1 June 2026 (the sliding-scale index raised to 992.24, source CNS).

The supplementary health insurance covers precisely this out-of-pocket. Four major players shape the market: DKV (the health entity of the LALUX Group), the CMCM mutual, AXA (OptiSoins) and Foyer (Medicis). They differ in their logic (private insurer vs mutual), their ceilings (optical, dental, laser), their cover abroad and their services (second medical opinion, assistance).

Hospital or full cover: what each plan actually covers

The levels of cover, and who each is for.
Level 1
Hospitalisation
Private room & stay

Covers the hospital stay, the private room, fee supplements and free choice of doctor. The essential to avoid a nasty surprise during an operation.

Level 2
Outpatient
+ Outpatient & alternative

Adds consultations, medicines, physio, alternative medicine (osteopathy, acupuncture) and prevention. For regular medical follow-up outside hospital.

Level 3
Full
+ Dental & optical

Adds dental (prostheses, implants, orthodontics), optical (lenses, frames, contacts) and laser surgery. The broadest cover, ideal for families.

The cover that makes the difference

Beyond the classic items, compare: the second medical opinion (DKV BEST CARE⁺, Foyer Best Doctors), the fast specialist appointment (DKV: within 5 working days), the dental and optical ceilings (CMCM: dental prostheses up to €3,500/year with DENTA & OPTIPLUS), refractive surgery (DKV EASY up to €3,000 from month 49, Foyer up to €1,000, CMCM D&O up to €1,000 per eye), and cover abroad (CMCM worldwide 24/7 assistance). These differences matter far more than price alone.

Our analysis of the 4 players

A comparative reading of the IPIDs and brochures — strengths and differentiation.

DKVDKV (Groupe LALUX)

The health entity of the LALUX Group, present in Luxembourg since 1981. The retail range is built on three cumulative covers: COMPACT (hospital and dental, offered with no age restriction), PLUS, which adds the whole outpatient side, and EASY HEALTH, the premium tier. Alongside them sit COMPLETE HEALTH, a comprehensive policy carrying an annual deductible, and HOSPITAL HEALTH, purely inpatient. The product leaflets place the gaps precisely: the daily allowance drops on EASY, but visual aids double from PLUS onwards and, above all, laser eye surgery becomes a benefit in its own right, up to €3,000 after four years on the books, where the other two tiers count it against the optical allowance. On dental, orthodontics runs up to €4,000 per insured where treatment starts before age 17. The service that makes the difference remains BEST CARE: a second medical opinion and surgery arranged with an expert within 5 working days, included from PLUS as is TRAVEL⁺. A rare detail on this market, EASY HEALTH carries the “Made in Luxembourg” label. Note: although the DKV brand is being integrated into its parent company ERGO in Germany, DKV Luxembourg continues its activities independently, with no consequences for its clients.

CMCMCMCM

The country's large mutual (since 1956, around 280,000 members). Its logic is unique: no medical questionnaire, no exclusion based on age or health, and co-affiliated family members are covered at no extra contribution, with a 10% discount for single under-30s. Three cumulative levels: Régime Commun, then PRESTAPLUS, then DENTA & OPTIPLUS, which pushes dental and optical to the top of the market — dental prostheses up to €3,500/year. The CMCM worldwide 24/7 assistance is included from the base, with hospitalisation abroad covered up to €100,000 per incident and unlimited repatriation. The July 2026 brochure confirms the announced increases and adds long-awaited items to PRESTAPLUS: psychological follow-up for minors, adult psychotherapy, orthopaedic insoles and lipoedema cover. Where the CMCM really stands apart is prevention, an area private insurers most often exclude: screenings, cardiovascular markers, vaccinations and dietetic follow-up are reimbursed from the Régime Commun, alongside the new Odyssée programme at Domaine Thermal de Mondorf. Note: benefits are provided pro rata within the mutual's available resources.

AXAAXA OptiSoins

The supplementary health cover of the international AXA group. The base covers hospitalisation-related medical costs, pre- and post-hospital care, and assistance abroad (rescue costs up to €10,000, medical assistance, repatriation or transport). A valuable feature: practical day-to-day services (home help, pet care, school support) during and after hospitalisation, granted on Luxembourg territory for insureds resident in Luxembourg. Depending on the chosen plan, the contract extends to outpatient care (consultations, medicines, spa cures, imaging, alternative care), prevention and screening, dental and eye care. Cover in Europe, and worldwide for temporary stays of up to 60 days. The contract runs for two years, renewable annually, with hospitalisation reported within 10 days.

FoyerFoyer Medicis

The supplementary cover of one of the country's main insurers, in two plans: medicis hospi+ (hospitalisation, private room, free choice of hospital across Europe, care 30 days before / 90 days after) and medicis confort (which adds outpatient care, optical and refractive surgery, dental with orthodontics up to €3,200 and alternative therapies at 80%, up to €1,000/year). Distinctive service: Best Doctors (second opinion from a global network) included in both plans, with 24/7 assistance and the MyFoyer app. Geographic scope: Europe for curative treatment, and worldwide during a temporary leisure or business trip, for a maximum of 8 consecutive weeks.

CMCM or private insurer: what is the difference?

Two very different logics — which suits you?
🤝
The mutual (CMCM)

Solidarity principle: no medical questionnaire, no exclusion based on age or health, family covered at no extra cost. In return, benefits are provided within the limits of available resources (they are not firm contractual guarantees as with an insurer). Ideal for families and profiles that private insurers price highly.

📑
The private insurer (DKV, AXA, Foyer)

Firm contractual cover and high ceilings (dental, optical, laser), premium services (second opinion, fast appointment). In return, a medical questionnaire, waiting periods and often an initial 2-year commitment. Ideal if you want precise ceilings and high-end services.

Which health insurance to choose: family, senior, self-employed?

Family, senior, self-employed: the criteria that change everything.

Family. The decisive factor is cost per person: CMCM covers co-members (spouse, children) at no extra cost — unique in Luxembourg — while private insurers (DKV, AXA, Foyer) price each insured person. For children, look at orthodontics: CMCM D&O reimburses €500/child before 17, DKV covers the diagnosis before 17. Singles under 30 also get a 10% CMCM discount.

Senior. The hard point is access to the contract: CMCM applies no medical questionnaire, no exam, and no age or health exclusion — you can join at any age, even with a medical history. At private insurers, the questionnaire conditions acceptance and premiums rise with age; DKV COMPACT remains accessible "without age reservation". Self-employed. The CNS covers the basics as for an employee, but hospital out-of-pocket costs weigh directly on your business: hospital cover (private room, €30/day allowance with DKV COMPACT) is the priority foundation — cover levels in the plans section.

How much does health insurance cost in Luxembourg?

Indicative ranges and tax relief — the contribution depends on age and plan.
🏥
Hospitalisation
~€20–40/mo

Private room and stay. The most affordable level.

🩺
Outpatient
~€40–70/mo

+ outpatient, medicines, alternative medicine.

🦷
Full
~€70–120/mo

+ dental, optical, laser. Ideal for families (or CMCM, family included).

The rate depends heavily on age at joining, the plan and the insurer. Because health spending rises roughly twice as fast as inflation and private insurers' premiums are age-based, joining young locks in a lower rate. The CMCM applies a solidarity-based contribution and covers the family at no extra cost, which changes the equation for a household. On tax, good news: supplementary health premiums are deductible as special expenses, up to €672 per person per year (article 111 LIR), a ceiling shared with other insurance. Each provider issues an annual tax certificate.

Medical questionnaire and waiting period: the conditions to join

The real divide between the mutual and private insurers.

CMCM (mutual): no medical questionnaire, no exam. Membership is open to everyone regardless of age or health — the trade-off is a 3-month waiting period (12 months on re-joining) before benefits are paid. DKV, AXA and Foyer (private insurers): mandatory medical questionnaire, with contractual waiting periods; your medical history can lead to exclusions or a surcharge. Never downplay a condition in the questionnaire: an inaccurate declaration can void cover exactly when you claim.

Commitment and geographic cover: DKV applies an initial 2-year commitment (30 days' notice), Foyer a 2-year term with tacit renewal. Geographically: DKV covers Europe (+ worldwide for the first month of a stay), Foyer covers Europe (stays ≤ 60 days outside Europe), while CMCM includes worldwide 24/7 assistance from the base level (hospitalisation abroad up to €100,000/incident, unlimited repatriation). Practical advice: sign up before you need it — after 50 or after a diagnosis, the mutual is often the only door left wide open.

Health insurance for cross-border workers: which side to insure?

Getting care on both sides of the border.

Cross-border workers affiliated to the CNS can subscribe to supplementary cover with all four players. Their specific issue is cover on both sides of the border (Luxembourg and country of residence). On this point: the CMCM offers worldwide 24/7 assistance from the base; Foyer guarantees free choice of hospital across Europe; DKV offers the TRAVEL⁺ option. Note: DKV and AXA contracts cover Europe (outside limited temporary stays), a point to check if you get care outside Europe. Tax deductibility only applies to taxpayers filing a return in Luxembourg.

Frequently asked questions about health insurance in Luxembourg

Everything you need to know about supplementary health insurance in Luxembourg.

Which supplementary health insurance to choose in Luxembourg?

In September 2026, the best choice is decided by profile. For a family, the CMCM mutual is the most advantageous: co-affiliated members (spouse, children) are covered at no extra cost and with no medical questionnaire — something no private insurer matches. For comfort and speed of care, DKV EASY (the health arm of the LALUX Group) stands out with its BEST CARE network (specialist appointment guaranteed within 5 working days, second medical opinion, €500 ancillary-costs lump sum) and the panel's only dedicated laser-surgery benefit — up to €3,000 from the 49th month of insurance. For dental and optical, CMCM D&O has the highest ceilings (prostheses up to €3,500/year, adult orthodontics €2,000), ahead of Foyer (orthodontics €3,200). For hospitalisation only at lower cost, Foyer medicis hospi+ (free choice of hospital across Europe) or DKV COMPACT (private room). For care abroad or a cross-border worker, CMCM is the most protective (worldwide 24/7 assistance, hospitalisation up to €100,000), where DKV and AXA are limited to Europe. For a profile with pre-existing conditions or no medical exam, CMCM accepts with no questionnaire and no age exclusion.

Which supplementary health insurance for a family?

For a family, the CMCM is often the most advantageous: co-affiliated members (spouse, children) are covered at no extra contribution and with no medical questionnaire, with extensive dental and optical cover. If you want high contractual ceilings and premium services (second opinion, fast appointment), DKV (LALUX Group) or Foyer Medicis are excellent options. Remember the tax-deductibility of premiums (ceiling 672 € per person).

Can cross-border workers subscribe?

Yes. Cross-border workers affiliated to the CNS can subscribe to supplementary cover with DKV, CMCM, AXA or Foyer. The key issue for them is cover on both sides of the border: the CMCM offers worldwide 24/7 assistance from the base, Foyer free choice of hospital across Europe, DKV a TRAVEL⁺ option.

Is supplementary health insurance mandatory in Luxembourg?

No. Every resident or worker is affiliated to the CNS, which reimburses most everyday care. Supplementary health insurance is optional, but strongly recommended to cover the out-of-pocket: private room, advanced dental and optical, alternative medicine and care abroad.

Which companies offer supplementary health insurance in Luxembourg?

The main players are DKV (the health entity of the LALUX Group), the CMCM mutual, AXA (OptiSoins) and Foyer (Medicis). DKV, AXA and Foyer are private insurers with a medical questionnaire; the CMCM is a solidarity-based mutual with no questionnaire that covers the family at no extra cost.

What does supplementary health insurance cover?

It covers the out-of-pocket left by the CNS: the private room and fee supplements in hospital, part of dental and optical, refractive (laser) surgery, alternative medicine and care abroad. The most complete plans add a second medical opinion and assistance.

What is the difference between the CMCM and a private insurer?

The CMCM is a mutual: no medical questionnaire, no exclusion based on age or health, and co-affiliated family members are covered at no extra contribution. Benefits are provided within the limits of available resources. Private insurers apply a questionnaire and waiting periods, but offer firm contractual cover and high ceilings.

How much does supplementary health insurance cost in Luxembourg?

The price depends on age, plan and insurer. A hospitalisation-only plan is the cheapest; a full plan (dental, optical, outpatient) costs more. The CMCM contribution is solidarity-based and the family is covered at no extra cost. Request a personalised quote for an exact rate.

Why take hospitalisation insurance?

In hospital, the CNS covers the standard room but leaves a personal contribution (€26.14 per day in 2025, excluding under-18s, up to 30 days per year). The private room and fee supplements are not covered by the CNS: this is precisely the role of hospitalisation insurance, which guarantees the private room and free choice of doctor.

Is supplementary health insurance tax-deductible?

Yes. Premiums are deductible as special expenses, up to €672 per person per year (article 111 LIR), a ceiling shared with other insurance. Each provider issues an annual tax certificate.

Is there a waiting period?

Yes, usually. Private insurers apply waiting periods during which certain benefits are not yet covered, plus an initial contractual commitment (often 2 years). The CMCM applies a waiting period of a few months on joining. Check these periods before subscribing.

What does the CNS reimburse and why take complementary health insurance?

The CNS (national health fund) covers most care but leaves an out-of-pocket share: about 12% on a consultation, a €26.14/day hospital co-payment (2025 CNS rate, indexed), the individual room not reimbursed (0%), and advanced dental only partly covered. Complementary health insurance covers exactly that remainder: private room, fee supplements, dental, optical, alternative medicine and care abroad.

Are dental and optical care covered by complementary health insurance?

Yes — it's one of the main benefits. Extended dental/optical plans (DKV, CMCM DENTA & OPTIPLUS, Foyer confort) cover dental prostheses and implants, orthodontics, lenses, frames, contacts and refractive surgery, with annual caps. At the CMCM, the DENTA & OPTIPLUS level lifts dental care to €1,000/year and dental prostheses to €3,500/year, with €500 per implant (capped at €2,000/year), while optical runs through set allowances (lenses €150/year, contacts €150/year, frames €30/year, refractive surgery €1,000 per eye). At DKV, orthodontics reaches €4,000 per insured on COMPACT and PLUS where treatment starts before age 17. Check the caps and waiting periods.

Does complementary health insurance cover care abroad?

It depends on the insurer. CMCM offers 24/7 worldwide assistance from the base cover (hospitalisation abroad up to €100,000/incident, repatriation). DKV and AXA cover Europe (and worldwide for short stays), Foyer covers Europe with free choice of hospital. This is key for cross-border workers and travellers: check the geographical scope before subscribing.

How to choose affordable top-up health insurance in Luxembourg?

The goal isn't the absolute cheapest but the right cover-to-price ratio for your needs: (1) a hospitalisation-only plan (private room) is the cheapest and covers the heaviest risk; (2) a full plan (outpatient, dental, optical) costs more but reimburses everyday care; (3) the CMCM (mutual fund, no medical questionnaire, family covered at no extra cost, 10% discount for single under-30s) is often the most accessible; (4) compare with DKV, Foyer and AXA. Premiums are tax-deductible (cap €672/year).

What out-of-pocket does the CNS reimburse above a threshold?

The CNS applies a cap on personal contributions: once your accumulated out-of-pocket for the year exceeds a set amount, it reimburses the excess on request. This threshold is set at €518.18 in 2026 (versus €509.68 in 2025 and €497.38 in 2024). The agreed tariffs were also indexed on 1 June 2026 (the sliding-scale index raised to 992.24). This, however, covers neither the private room nor fee supplements — hence the value of supplementary health insurance.

What is new at DKV and the CMCM in 2026-2027?

At DKV, the retail range reads as three cumulative tiers — COMPACT (hospital + dental, no age restriction), PLUS (which adds outpatient) and EASY HEALTH (the premium tier) — completed by COMPLETE HEALTH (comprehensive insurance, annual deductible of €400 up to age 19 and €800 beyond) and HOSPITAL HEALTH (pure hospital cover). The product leaflets quantify the gaps precisely: a daily allowance of €30/day on COMPACT and PLUS against €20/day on EASY; visual aids of €250 every 2 years on COMPACT against €500 on PLUS and EASY; laser eye surgery up to €2,000, rising to €3,000 from the 49th month of insurance but reserved for EASY; alternative treatments reimbursed at 80% up to €500/year on PLUS and €1,000/year on EASY; orthodontics up to €4,000 per insured on COMPACT and PLUS where treatment starts before age 17; professional scaling up to €75/year throughout and the implant priced separately on EASY (up to €600). BEST CARE⁺ and TRAVEL⁺ are included from PLUS (optional on COMPACT). The waiver of the 3-month waiting period, valid until 1 August 2026, has now ended. DKV Luxembourg stays on the Luxembourg market: a 100% subsidiary of the LALUX Group since 2015 and run independently, it continues its activities even though the DKV brand is being integrated into its parent company ERGO in Germany. At the CMCM, the July 2026 benefits brochure confirms the announced increases — hospitalisation abroad raised to €100,000 per member per incident, frame allowance now €30 per calendar year — and documents new PRESTAPLUS benefits: psychological follow-up for under-18s (€100/year), adult psychotherapy (€40/year), orthopaedic insoles (€150/year), ambulance transport in Luxembourg, and an emergency consultation abroad covered up to €2,500/year. New partnership: the 24-week Odyssée programme at Domaine Thermal de Mondorf, reimbursed up to €400.

Can you get health insurance without a medical questionnaire in Luxembourg?

Yes — that is CMCM's specificity: no medical questionnaire, no exam, no age or health exclusion, with a 3-month waiting period in return (12 months on re-joining). Private insurers (DKV, AXA, Foyer) all require a medical questionnaire that conditions acceptance and pricing, with contractual waiting periods. After 50 or with a medical history, the mutual is often the only door left wide open — sign up before you need it.
